You will effectively be able to screen and assess the individual and family needs. You will be highly skilled in case planning by understanding needs and goals of clients. Maintaining an empathetic and professional approach to all cases whilst maintaining confidentiality and privacy requirements.Must have a thorough understanding of risk assessment and the ability to coordinate with management, accounts, clinical coordinators and case managers. The Person:
Allied health background particularly community aged care
Knowledge of Community, Aged Care and Disability care standards, guidelines and packages
Ability to manage your own caseload

Clean drivers licence
Tertiary qualified
